Dr. David Blackman
Dr. David Blackman is a board-certified and licensed dentist with over 35 years of experience in general, cosmetic and family dentistry. Dr. Blackman earned a Bachelor of Science degree in Biology from the University of Illinois at Chicago (UIC) and is a graduate of the College of Dentistry at UIC where he received his Doctorate in Dental Science.
Dr. Blackman’s approach to dentistry combines decades of experience with an eye for detail, resulting in a beautiful and healthy smile. A customized treatment plan is created for each patient to ensure a smooth experience from start to finish. This emphasis on personal attention and individualized treatment stems from Dr. Blackman’s commitment of providing patients with the best dental care.

When he’s not at work, Dr. Blackman enjoys spending time with his wife and family. Dr. Blackman has been married to Patty for 35 happy years. They have 3 grown children. His oldest, Carolyn, is married to Andy. Dr. Blackman just became a first-time grandpa to Carolyn and Andy’s daughter. Michelle is married to Justin, who is currently serving our country in the Army. Dr. Blackman’s youngest, his son Mark, is a junior at Marquette University.
Dr. Patrick Carroll, DDS – Oral Surgeon
Dr. Patrick Carroll is an Oral & Maxillofacial Surgeon originally from the Chicagoland area. Dr. Carroll completed his undergraduate studies at Indiana University, Bloomington in 2003 with honors and earned an early admission to dental school. While at Indiana University School of Dentistry, he developed an interest in surgery. He graduated with distinction in the top 10% of his class in 2007. Then he began a hospital-based residency program in Oral & Maxillofacial Surgery at Vanderbilt University Medical Center in Nashville, TN where he completed his training in 2011. Vanderbilt University Medical Center is a tertiary care facility and Level-1 Trauma Center where Dr. Carroll was an integral part of the facial trauma and reconstruction team. In addition to various rotations in general surgery, anesthesia, and internal medicine, he was exposed to congenital deformities such as cleft lip/palate, head and neck pathology, facial cosmetics, orthognathic corrective jaw surgery, TMJ disorders, and all aspects of dentoalveolar surgery including dental implants and wisdom teeth.
Dr. Carroll enjoys private practice and impacting his patients lives in a positive way. In his free time, he enjoys traveling and exploring the great city of Chicago.
Joseph Kizior, DDS – Orthodontist
Dr. Joseph E Kizior received his doctor of dental surgery with honors from Loyola University Dental School – Chicago, Illinois. Dr, Kizior also completed an advanced specialty program at Loyola University receiving a Master of Science degree in oral biology and a certificate of specialty in orthodontics. The research program Dr. Kizior worked on concentrated on the sensory (pain) receptors in the ligament that surrounds the roots of the teeth. The findings were published in the journal of Dental Research, titled “The Sensory Receptors of the Periodontal Ligament”, and they have clinical orthodontic applications to this day.
After receiving this specialty license, Dr. Kizior started his private practice. Shortly after that, he received an appointment as clinical professor in orthodontics at Loyola University. This enabled him to be familiar with the latest research and treatment techniques in orthodontics.
